Your text might depend on the athlete’s age, the sport and the level of competition.

The particular questions are less important than opening the lines of communication.  

Simply showing you care enough to ask will start your relationships off on a great foot.

In order to be the best coach I can for each athlete and for the team, I need to know what makes [you/your athletes] tick, both on and off the field [court/pool/track/etc.].  

Your responses to this short questionnaire will help me do that.  The more you are comfortable sharing the better.  

Simply reply to this e-mail and type your answers under the questions.  I will of course keep all of your information confidential.

Also feel free to reach out at any time.  This is just a start to the conversation that I hope will make this season a great experience for everyone.
